The Polk County Sheriff’s Office offers online report requests to individuals and companies. To initiate a Public Records Request, please contact Records for the necessary forms. Once the forms are filled out simply send them to the Polk County Sheriff’s Office Records Management Bureau.

Once your records request has been electronically received you will receive
notice from a Records staff member to notify you if any more information is
required to help expedite your request and to let you know when your request is
ready. Please be aware any request totaling over $30.00 will need payment
before being filled.
